Welcome to the Scholarship Application!  

We want to invest in the next generation of leaders who Make a difference daily inspiring everyone.

Open to high school seniors and graduates who have been accepted to or are currently enrolled in an accredited two- or four-year college, university, or trade school.

SUBMISSION OPTIONS:

• Short Essay 500-800 words
• Personal Testimony Video 3-5 minutes

• What High School do you currently attend
• What college you have been accepted to & plan to attend

Option 2 if you’re nominating a student:

• Your Name
• Your contact information
• Students Name & Contact information (if you have it)
• High School the student attends 
• Your relationship to student.

Short Essay/ Personal Testimony Video Prompts

• How your church/youth group has impacted your life. 
• Give your Testimony. 
• Briefly describe a time you have shown “Love Like Maddie.”

If you are nominating a student share why you believe this student reflects the heart of the M.A.D.D.I.E Scholarship and how they have shown love, kindness, And made a different in the lives of others.

Your essay is not just an application requirement, it’s an opportunity to tell your story, reflect on your journey, and share how your involvement in your church youth group or community has impacted your life. Whether you’ve organized charity events, volunteered, or simply been there for a friend in need, we want to know how you’ve made a difference. 

No story is too small or insignificant as one not shared.
